Para Eyes is a horror game where the core gameplay is built around finding differences while monitoring security systems. The central task requires viewing multiple live camera feeds simultaneously, carefully scanning these images for hidden or obvious anomalies that disrupt the normal environment. The protagonist must identify discrepancies, from out-of-place objects to more disturbing changes in the observed rooms. Success depends not only on finding these oddities, but also on managing the character's deteriorating mental state, which requires keeping an ear to the ground at all times.

Surviving the night shift is the primary goal in Para Eyes, which is achieved by accurately identifying and reporting unusual occurrences on screen. Failure to cope with the mental load or accurately reporting observed phenomena leads to negative results. Progression in this project brings rewards; they provide access to new locations for observation, presented as levels, each of which may contain unique design features and layouts. The gamer also unlocks higher difficulty levels and encounters an expanded set of abnormal events, testing their acuity and short-term memory.
