Netherworld Covenant places the player in the abyss, where the mortal world and the underworld intertwine after a catastrophe. As a survivor, the character wields the Lantern of the Void, an artifact that connects the two dimensions. This item lights up with azure flames, allowing the player to traverse procedurally generated labyrinths and destroy cursed barriers. The goal is to confront fallen heroes and demonic overlords to uncover the truth woven from blood and soul. In this world, the lantern serves as a conduit for summoning the fallen.

The combat system in Netherworld Covenant emphasizes tactical actions rather than rapid combinations of attacks. The protagonist must use parry, dodge, and Ethereal Dash to avoid damage before performing precise counterattacks. The key gameplay feature is the Spirit Companions system, which opens access to 4 ghostly allies: the Swordsman, the Archer, the Guardian, and the Rogue. These summoned souls help by passing through obstacles for surprise attacks, creating tactical advantages or moving their owner between worlds to avoid a fatal blow.

The opponents are fallen lords and once noble knights, whose bodies are distorted by the terrible energy of souls. In between forays into dangerous lands, this game allows you to return to the border camp. In this refuge, spectral allies are gathered, weapons are created from demonic blood and materials are accumulated for the next attempt. Through scraps of dialogue, the truth about the cataclysm is revealed, but each trip carries the risk that the name of the lantern’s owner will become another epitaph on the Stone of the Covenant.
