In Chasmal Fear, enemies have a special feature: some defeated creatures are able to unpredictably transform into more dangerous versions, changing previously cleared locations. The mutation system requires finishing off enemies before they begin to transform. The decisions made directly shape the further path; choosing one route often makes others inaccessible, affecting subsequent skirmishes and available tactical options, from direct combat to using cover. The title supports single-player mode or co-op with a partner, adjusting the difficulty to the gamer's playing style.

Exploration involves more than just moving along corridors; The player must uncover the backstory of the complex by finding and interpreting various clues: emails, audio recordings, and video logs.
