Esophaguys features characters who are elderly people with the ability to extend their necks. Players control these extendable necks to bite, grab, and move around locations, working together or against each other. The title supports from 1 to 4 people, depending on the selected activity. The action unfolds to a soundtrack of ringing tones and old groans, creating an auditory background for all gameplay.

The cooperative component of Esophaguys invites 2-4 players to work together to explore the environment and solve puzzles to reactivate ancient monuments. The competitive part invites gamers to engage in direct competition in team competitions or solo battles in the arena. A separate masochistic mode removes checkpoints and solid ground, presenting a series of tests in an environment designed for speed runs and testing endurance.

Story Mode is designed for 1-4 players and invites you to embark on a narrative journey to reunite a lost people. This adventure is accompanied by a narrator and explores the nature of the heroes' physical features. Completing all game modes contributes to the development of the central hub world, where everyone works together to restore a monument. Completing these tasks unlocks access to the kin's attire, allowing you to customize the heroes' appearance with the attire of their forgotten relatives.
