Otherskin sends protagonist Alex on a solo mission to the alien planet Vandermere, the suspected source of a cosmic threat called the Corruption. Alex is the 13th Space Crusader sent on this expedition, equipped with a high-tech AI suit to gather information before the wearer succumbs to the planet's dangers. The goal of each raid is to extend the time of survival on the surface to provide scientists with more data to find a solution.

The central gameplay mechanic allows the heroine to absorb alien morphs from enemies, opening up access to new moves and abilities. Otherskin's gameplay is built around upgrading a unique AI suit that provides support throughout the journey. Upgrades allow you to maximize the impact of weapons, the effectiveness of life support systems, and maneuverability in combat. The combat system combines the use of powerful long-range weapons with quick melee attacks.

The player can freely switch between these two combat styles when fighting enemies, allowing for flexible responses to the various threats encountered on Vandermere. Exploration is a key aspect of the game, as the heroine must scout out the alien planet to learn more about the origins of the Corruption. Vandermere is made up of a variety of biomes, with each location offering unique challenges and information to gather. Carefully exploring every corner of this alien world is necessary to reconstruct the history of the extinct Magna species and understand the source of the growing threat.
