Tokyo Underground Killer follows Kobayashi, a mercenary nicknamed the Shinjuku Vampire, as he works in the criminal underworld of the metropolis. The character takes on contracts to eliminate dangerous maniacs using a sword, unique skills, and powerful weapons. The key task is to stop the invasion of the brutal Flatliners group. The basis of the combat system is tied to the management of blood energy, which feeds the killer’s mystical skills and powers during intense fights.

Tokyo Underground Killer merciless first-person combat focuses on maneuvers with a katana to slash, stab, and parry hordes of monsters inspired by Japanese demonology. A set of 10 different bloody skills provides tactical options for defeating difficult opponents. After a successful fight, absorbing the blood of defeated enemies replenishes both health and energy for using these skills. These violent encounters take place in surreal, neon-drenched versions of real-life neighborhoods like Shinjuku, Akihabara, and Shibuya.

In between missions, the player can customize their personal apartment in Shinjuku by purchasing decorations for it. The central city hub also provides access to various minigames to distract from the main storyline. These activities serve as a prelude to the protagonist’s leveling up, which is implemented through a skill tree. The entire gameplay is built around making Kobayashi stronger for future contracts, unlocking new abilities to combat urban threats.
